What is All-on-4™?
At Bangkok Smile Dental Clinic, the All-on-4™ protocol is a new approach to full-arch rehabilitation that lets you leave our clinic with a whole set of permanent, fixed teeth in just one trip. The technique uses four strategically placed implants — including tilted posterior implants — to support an acrylic bridge that can be fabricated and functioning within hours of surgery.
Around the world more than 300 million people are missing one or more teeth. Being edentulous (toothless) is a more common handicap than many realise. Decades of documented research have proven the long-lasting results of dental implants, particularly the physical benefits of osseointegration and bone preservation — and All-on-4 makes those benefits accessible in a single, predictable procedure.
The All-on-4™ concept changes what has become “traditional” thinking around immediate loading for the edentulous patient. It has near-universal application — a simplified, predictable approach that makes a full-arch fixed restoration affordable for a far greater segment of patients than before.

Is this treatment widely available?
No. Immediate loading with the All-on-4 concept is a relatively advanced area of implant dentistry. Only some clinics around the world offer it, because it requires very experienced surgeons, prosthodontists and lab technicians with advanced training in implant placement and immediate prosthodontics. The equipment is technologically sophisticated, the materials are intricate and valuable, and every step is held to meticulous sterile detail.
The Bangkok Smile All-on-4 process
Providing patients with Immediate Function is not new for our clinic — meticulous engineering of the prosthesis allows our patients to smile and chew while healing, with full fixed teeth in place within three days of surgery. Here’s how each stage looks:
- 3D consultation & digital planning — a CBCT scan maps your bone, sinuses and nerves. Your case is designed digitally with NobelGuide™ so every implant position is determined before you sit in the surgical chair.
- Implant surgery (Day 1) — four implants are placed per arch under local anaesthetic, with the posterior implants tilted to capture the strongest available bone. Most cases are flapless and painless.
- Immediate temporary bridge (Day 1–2) — within hours of surgery, an acrylic bridge is fitted to the implants. You leave the clinic with fixed teeth, chewing and smiling immediately.
- Healing & osseointegration — the implants integrate with the bone over the following months. You eat, speak and function normally throughout this period.
- Final permanent bridge — once integration is confirmed, individual Procera® crowns in Alumina or Zirconia are cemented onto a Procera® bridge framework for a beautiful, lasting result.

By its nature, dental implant treatment is a significant investment. Can I really leave with fixed teeth on the same day?
Yes — that’s the defining feature of All-on-4. Within hours of the implant surgery an acrylic bridge is attached to the implants, so you leave the clinic chewing and smiling on a complete set of fixed teeth. The temporary bridge functions throughout healing, and the final Procera® porcelain bridge is fitted once the implants have integrated with the bone.
Will I need a bone graft?
In most cases, no. All-on-4 is specifically designed to maximise the use of available bone — the posterior implants are tilted to capture the strongest existing bone, which usually eliminates the need for grafting. The 3D CBCT scan at consultation tells us with certainty.
Is the surgery painful?
The surgery itself is painless under local anaesthetic, and when combined with NobelGuide™ it is also flapless — meaning no cutting and lifting of gum tissue. Most patients describe recovery as much milder than expected, and the relief of having fixed teeth straight away usually outweighs any post-operative soreness.
How long does the whole process take?
For most international patients, the entire treatment fits into a single trip of about 5–7 days: consultation and 3D planning, implant surgery, temporary bridge fitting and post-operative review. The final permanent porcelain bridge is delivered on a follow-up visit a few months later, once integration is complete.
